Wonder what happened? Looks like they had layed on the lock tight on the nut threads and set screws.....and it still came off.
And now I fixed it....say what you want,but it won't be coming off without a grinder.

I found a slick set up, may have seen it on here, I can't remember. It looks like a socket with splines instead of the square drive. It goes on the nut and the hole on the splines, locking the 2 together. Last I knew it was about $175 for it, not bad considering the price of a main shaft!
